How to Apply for a Replacement Social Security Card
You can apply for a replacement social security card online, by mail or in person at your local social security office. There is no charge for this service.
Apply online
You can apply for a replacement Social Security card online if you have a Social Security account, and if you:
- Are a U.S. citizen age 18 or older with a U.S. mailing address (this includes APO, FPO, and DPO addresses)
- Are not requesting a name change or any other change to your card; and
- Have a driver's license or state-issued identification card from one of the many participating states or the District of Columbia.
Apply in person at a Social Security office that serve your area
Visit the SSA website to find your local social security office
You will need to:
- Fill out Form SS-5 Application for a Social Security Card
- Bring the following original documents:
- Evidence of citizenship –
- U.S. birth certificate or
- U.S. passport
- Certificate of Citizenship or
- Certificate of Naturalization
- Evidence of age –
- U.S. birth certificate or
- U.S. passport
- Evidence of identity –
- U.S. driver's license or
- U.S. State-issued non-driver identity card or
- U.S. passport
- Evidence of citizenship –
Apply by mail:
Mail to any Social Security Office. Visit the SSA website to find your local social security office
Mail signed Form SS-5 Application for a Social Security Card, and The following original documents (Social Security office will return any documents you mail to them)
- Driver's license or ID card, and
- U.S. Birth certificate or U.S. passport, or Certificate of Citizenship or Certificate of Naturalization
